Cup not preheated: Preheat the cup.

Milk not heated enough (for milk coffee): Stop when the milk jug bottom becomes too hot to touch.

Cold coffee grounds or beans: Allow them to reach room temperature before use.

Solutions: Preheat the cup; heat the milk properly; use room temperature coffee grounds/beans.
